1. Unique ID#: number (not letters) code located immediately above your name on your mailing label that starts and ends with a #-sign (not the letters #BXNRPPL, which is our company's ID#, not yours, which is on the next line down).
2. Name
3. Address
4. Day phone number (for USPS Periodicals audits only)
5. Signature (not required on emails)
6. Date
7. Statement: "Please renew my free 3-year subscription to NCboatinglifestyle magazine".
